What a Real M 30 Pill Is

The tablet counterfeiters are copying is oxycodone hydrochloride 30 mg, a prescription opioid for severe pain. It is blue, round, and stamped M on one side and 30 on the other.

Immediate-release oxycodone is made in 5, 10, 15, 20 and 30 mg, so the M 30 is the top of that range. It is a Schedule II controlled substance, and legitimate tablets come from a licensed pharmacy against a prescription.

Before counterfeits enter the picture, the imprint M 30 is already shared.

Searching it returns five products. Oxycodone hydrochloride 30 mg is blue and round. Clorazepate dipotassium 3.75 mg is also blue and also round. Amphetamine and dextroamphetamine 30 mg is white and eight-sided. Morphine sulfate 30 mg is white and round. A chewable lisdexamfetamine 30 mg is white and three-sided.

The first two are the problem. Clorazepate is a benzodiazepine prescribed for anxiety and alcohol withdrawal, not an opioid, and 3.75 mg of it is nothing like 30 mg of oxycodone. Both tablets are blue, both are round, and both read M 30.

So a blue round M 30 found loose is not automatically oxycodone even when it is entirely legitimate. That is worth knowing before the counterfeit question is even raised.

Why Appearance Cannot Tell You Anything

Counterfeit tablets are pressed on equipment that reproduces the imprint, the color, the round shape, the size and the score lines. Some are close enough that people who take oxycodone regularly cannot pick them out.

The DEA's guidance on this is unusually blunt for a federal agency. Never trust your own eyes to determine if a pill is legitimate. The only medications the agency considers safe are those prescribed by a trusted medical professional and dispensed by a licensed pharmacist.

The scale behind that advice is large. In 2025 the DEA seized more than 47 million fentanyl-laced counterfeit pills along with nearly 10,000 pounds of fentanyl powder.

Why There Is No Safe Dose of a Counterfeit

Illegal presses do not follow pharmaceutical dosing standards, and nothing about the process produces consistency. Two tablets pressed in the same batch can contain different substances, or the same substance in very different amounts.

That variability is the whole danger, and the numbers explain why. Two milligrams of fentanyl is a potentially deadly dose. Fentanyl is 50 to 100 times more potent than morphine, which is why a quantity too small to see reliably changes a tablet from survivable to fatal.

Carfentanil, which also turns up in the illicit supply, is further out again. The DEA describes it as 100 times stronger than fentanyl and 10,000 times more potent than morphine, and says a nearly microscopic amount can produce a deadly effect in humans. It was developed as a tranquilizer for large animals and was never intended for people.

None of that can be judged by eye, by taste, or by breaking a tablet open.

How Real and Counterfeit M 30 Pills Actually Differ

Not by appearance. The differences that exist are all about where the tablet came from and how it reached you.

A legitimate tablet arrives in a labeled prescription bottle from a licensed pharmacy, with your name on it, a stated drug and a stated strength. It was made under quality control that keeps the amount in each tablet consistent.

A counterfeit arrives loose, in a bag, or in an unlabeled container, from a source that cannot be verified. There is no stated strength that means anything, no consistency between tablets, and no way to establish what is in it.

That is the only reliable test available to a person holding a pill. Provenance, not appearance.

Why Fentanyl Specifically

Fentanyl dominates the counterfeit supply for economic reasons rather than pharmacological ones. It is cheap to obtain and extraordinarily potent, so a small quantity produces an enormous number of tablets. That combination makes it profitable to press at scale with very little upfront cost.

The consequence shows up in the mortality data. Nearly 73,000 drug overdose deaths in 2023 involved synthetic opioids other than methadone, the category fentanyl belongs to.

What to Do If Someone Took One

Treat it as an emergency rather than waiting to see whether it gets worse. That is particularly true if the person is already hard to wake, breathing slowly, or unresponsive.

Call 911 first. Give naloxone as soon as possible if it is available and follow the package instructions. Stay with the person and keep watching their breathing until responders arrive. If they stop breathing and you are trained, begin rescue breathing or CPR.

Naloxone reverses an opioid overdose temporarily and then wears off, so emergency care is still needed even if the person wakes and seems fine. It causes no harm if opioids turn out not to be involved, so there is no reason to weigh the decision.
