Xanax, the brand name for alprazolam, is a prescription benzodiazepine used to treat anxiety and panic disorder. Because it slows activity in the central nervous system, it interacts dangerously with alcohol, opioids, sleep medications, muscle relaxers, supplements, and a number of prescription drugs. Knowing which combinations to avoid lowers your risk of severe sedation, slowed breathing, overdose, and other serious effects.
How Xanax Interactions Happen
Xanax increases the calming effect of gamma-aminobutyric acid, or GABA, which slows activity in the brain and nervous system and reduces anxiety and panic symptoms.1
That same mechanism is why interactions matter. Combine Xanax with anything else that slows the central nervous system and the effects stack: extreme drowsiness, confusion, poor coordination, slowed breathing, loss of consciousness, overdose.
A second class of interaction runs through metabolism. Alprazolam is processed by the CYP3A enzyme system. Some medications block that enzyme and push Xanax levels up. Others speed it up and pull levels down, making symptoms harder to control.1
Xanax and Opioids
This is the most serious interaction on the list. Opioids include prescription pain medications such as oxycodone, hydrocodone, morphine, and fentanyl, as well as illicit opioids like heroin.

The FDA warns that taking opioids with benzodiazepines or other CNS depressants, alcohol included, can cause extreme sleepiness, slowed breathing, coma, and death.2 NIDA reports that people using opioids and benzodiazepines together face higher rates of emergency department visits, hospital admission, and overdose death.3
Xanax and Alcohol
Alcohol should generally be avoided entirely. Both substances slow the brain and body, and combined their effects become stronger and far less predictable.
Alcohol can also worsen anxiety, sleep problems, depression, and irritability. If you take Xanax, talk to your prescriber before drinking. For most people, avoiding alcohol entirely is the safer answer.
Xanax and Other Sedatives
Xanax interacts with many medications that cause sleepiness. These combinations raise the risk of falls, confusion, accidents, impaired driving, and overdose.

Xanax and Antidepressants
Some antidepressants interact with Xanax. Sometimes the issue is added drowsiness or impaired coordination; sometimes the antidepressant changes how alprazolam is metabolized.
SSRIs can increase impairment when taken alongside Xanax:
Sertraline: may add to drowsiness or coordination problems
Fluoxetine: may increase central nervous system side effects
Paroxetine: may worsen sedation or cognitive slowing
Escitalopram or citalopram: may increase sleepiness or reduce alertness
Fluvoxamine matters most here, because it increases alprazolam exposure by affecting metabolism. Xanax labeling notes that a dose reduction may be needed.1
None of this means antidepressants and Xanax can never be combined. It means the combination belongs under a prescriber's supervision, particularly when starting, stopping, or changing doses.
Xanax and CYP3A4 Inhibitors
Alprazolam is metabolized mainly through CYP3A. Medications that strongly inhibit that pathway push alprazolam levels up and raise the risk of stronger, longer-lasting side effects.1
Ketoconazole and itraconazole are contraindicated with alprazolam, because they significantly impair CYP3A-mediated metabolism.1 Other inhibitors call for caution:
Azole antifungals: ketoconazole and itraconazole are the most concerning
Macrolide antibiotics: clarithromycin and erythromycin can raise alprazolam levels
HIV medications: ritonavir affects alprazolam exposure in complex, time-dependent ways
Cimetidine: this acid reducer may increase benzodiazepine effects
Nefazodone: this antidepressant may raise alprazolam levels
If you are prescribed an antibiotic, antifungal, antiviral, or new psychiatric medication while taking Xanax, ask whether it affects alprazolam metabolism.
Xanax and CYP3A4 Inducers
Other medications do the opposite, speeding up metabolism, lowering Xanax levels, and reducing its effectiveness:
Carbamazepine: may lower alprazolam exposure and weaken symptom control
Phenytoin: may increase metabolism of some medications
Phenobarbital: may reduce alprazolam levels through enzyme induction
St. John's wort: this supplement reduces levels of many medications by inducing drug-metabolizing enzymes
If levels drop too far, anxiety or panic can worsen. Do not compensate by taking more Xanax unless your prescriber tells you to.
Xanax and Grapefruit
Grapefruit and grapefruit juice interfere with enzymes that break down certain medicines, which can leave too much of a drug in the body.4
The grapefruit and alprazolam interaction is less clear-cut than some others, and caution is still reasonable. If you drink grapefruit juice regularly, or suddenly start drinking more of it, ask your prescriber or pharmacist whether it affects your dose or side effect risk.
Consistency is the safest approach. Do not make sudden diet changes involving grapefruit without checking first.
Xanax and Supplements
Supplements interact with prescription medications, natural label or not. With Xanax, some increase sedation and others change how the medication is processed.
Include supplements, herbal products, vitamins, and over-the-counter sleep aids whenever you give a provider your medication list.
Medical Conditions That Raise the Risk
Interactions are not the only concern. Certain conditions change how Xanax affects you:
Breathing problems: COPD or sleep apnea increase the risk when sedatives are involved
Liver disease: alprazolam is processed by the liver, so impairment intensifies side effects
Older age: greater sensitivity to sedation, confusion, and falls
Narrow-angle glaucoma: alprazolam is contraindicated in acute narrow-angle glaucoma1
Substance use history: raises the risk of misuse, dependence, and overdose
None of these automatically rules out Xanax. They do mean your prescriber needs the full picture before deciding what is safest.
How to Reduce Your Risk
If Xanax stops helping, talk to your prescriber. Tolerance, worsening anxiety, an interaction, or dependence all need addressing safely.
